서론
1990년 국민 건강 조사 자료에 의하면 15일간 상병율은 인구 1000명 당 263명이며 이들중
75%가 만성질환자였다. 그리고 만성질환자중 30%가 관절염 환자로서 국민 건강 유지상 관
절염 관리는 중요한 과제임에 틀림없다.
관절염은 질병 의 속도를 늦추거나 관해상태로 이끄는 표준 처방이 있음에도 불구하고 치
료가 어렵기 때문에 다양한 치료행위를 보이고 있다. 이렇듯 여러 치료 요법을 추구하는 이
유는 장기간의 통증 경험과 퇴화과정을 나타내기 때문이고 단기간내 치료 효과가 나타나지
않거나 부작용이 있으면 이에 실망하여 치료 요법을 중단하든지 흑은 치료 방법을 전환하기
때문이다. 또 다른 이유는 의료인이 환자와의 신뢰관계 형성에 실패함으로써 지속적인 치료
를 유도하지 못하고 있다는 점이다. 이렇게 하여 변환된 치료 방법은 비 전문인에 의한 권
유와 생활 경험으로부터 구전되고 치료 효능이 검증된 바 없는 민속 요법이 대부분이다. 그
러므로 이러한 비효능적인 치료과정을 반복하는 가운데 환자는 치료시기를 놓치게 되어 경
제적인 면에서나 치료의 효율적인 측면에서 손실을 입게 된다.
관절염환자의 통증 경험에 관한 은영의 연구에 의하면 환자들은 구전에 의한 민속요법을
적어도 1회이상 사용한 경험이 있다고 보고하였다. 대부분의 환자들은 주치의와 상의하지
않고 민속요법을 사용하고 있어서 민속요법이 기존 치료의 대안일 수도 있고 초기부터 선택
가능성이 있는 방법일 수도 있다. 그러나 민속요법은 약효의 신뢰성 문제 이외에도 의료보
험의 혜택을 받지 못하여 치료비용의 과중함과 질병치료의 지속성 유지가 어렵다는 문제를
야기하기 때문에 그 영향을 긍정적으로 평가하기는 어렵다. 뿐만 아니라 민속요법에 드는
비용은 매월 253,583원이었고, 관절염 병원치료에 소요되는 비용은 78,380원으로서 민속요법
에 드는 비용이 훨씬 크다. 또한 간호는 환자들의 바람직한 건강행위를 증진시키고 잘못된
건강 행위를 변화시키는 촉매 역할을 해야 함은 주지의 사실이다. 그러므로 관절염 환자들
의 치료 전이행위에 대한 의사결정 과정을 이해하고 무절제한 민속요법의 사용을 피하기 위
해서는 민속요법 사용에 대한 실태를 파악함과 동시에 그 선행요인을 파악하는 연구가 필요
하다. 본 연구에서는 치료행위 과정을 기술하고 이를 예측할 수 있는 영향 변수간의 관계를
파악하여 치료행위 선택의 모형을 구축하고자 실시되었다.

This study was undertaken to explore the antecedent factors and process of the
treatment-seeking behaviors of medical and alternative treatments in patients with
arthritis using methodological triangulation.
The data were collected from 995 arthritic patients who were registered either in a
center of rheumatology for medical treatment or residents of community having no
treatment to classify different treatment patterns. Sixteen patients with various types of
treatment only, alternative treatment only, and no treatment were selected among the
total samples to identify the antecedent factors through in-depth interview.
The quantitative data were analyzed by percentile, t-test, chi-square test and
discrimant analysis using SAS PC program, while the qualitative data were analyzed by
means of grounded theory methodology.
Treatment-seeking behaviors of patients change from the early stage to the sick-role
stage. At the early stage, initial characteristics of pain and acculturation of medical
professionalism affect the choice of treatment patterns. The acculturation of medical
professionalism is affected by health care accessibility, level of education, duration of
sickness and lay referral system. At the sick-role stage, lay referral system and
acculturation of medical professionalism affect the choice of treatment patterns. The
acculturation of medical professionalism is affected by characteristics of symtoms,
perceived treatment effects, perceived causes of diseases and socio-economic status as
well as health care accessibility, level of education and lay referral system.
In conclusion, different factors as well as common factors are influencing the
treatment-seeking behaviors depending on the disease and treatment stages.
More detailed further studies are required to explore the value system or medical
acculturation of patients which is one of the most important factors In decision-making
about treatment modalities.
